چکیده

UNLABELLED A young man underwent 2 catheter ablation procedures because of frequent episodes of paroxysmal supraventricular tachycardia (SVT). A left accessory pathway was ablated in the first procedure. The patient underwent the second ablation procedure due to a recurrent SVT demonstrated by 12-lead electrocardiogram two weeks after the first procedure. During the second procedure, a SVT with a different retrograde atrial activation sequence was present. Programmed stimulation at the right ventricular apex showed multiple retrograde atrial activation sequences. Thereafter, careful analysis of the intracardiac electrograms helped us to successfully ablate the SVT. KEY WORDS Ablation; Electrophysiology; Supraventricular tachycardia.

A Narrow QRS Complex Tachycardia With An Apparently Concentric Retrograde Atrial Activation Sequence

The retrograde atrial activation sequence constitutes an initial important clue to elucidate the tachycardia mechanism during diagnostic electrophysiological testing in patients with supraventricular tachycardia. However, in some cases its correct analysis is challenging.

Supraventricular tachycardia with cycle length variation and apparent VA dissociation: what is the mechanism?

We report on a 33-year-old woman who underwent an electrophysiologic study because of a documented supraventricular tachycardia. The induced tachycardia presented with cycle length variations and an apparent ventriculo atrial (VA) dissociation on the surface ECG. Careful analysis of the activation sequence revealed the correct diagnosis of an atrioventricular nodal re-entrant tachycardia using ...

Catheter mapping of retrograde atrial activation. Observations during ventricular pacing and AV nodal re-entrant paroxysmal tachycardia.

A systematic study of retrograde atrial sequence at commonly used electrode catheter recording sites in 8 patients without, and in 4 patients with AV nodal re-entrant paroxysmal tachycardia was made. During right ventricular pacing, the retrograde atrial activation sequence was low septal right atrium--proximal coronary sinus--distal coronary sinus--high right atrium. During the episodes of par...

Narrow QRS Tachycardia with Alternate Wide QRS Beats: What is the Mechanism?

A 44-year old lady underwent electrophysiology study for recurrent palpitations and documented narrow QRS regular tachycardia. The baseline ECG showed subtle preexcitation that was easily manifest on atrial pacing. The retrograde atrial activation sequence during ventricular pacing was eccentric suggesting retrograde conduction over the accessory pathway. A regular narrow QRS tachycardia with c...

Intractable paroxysmal tachycardia caused by a concealed retrogradely conducting Kent bundle. Demonstration by epicardial mapping and cure of tachycardias by surgical interruption of the His bundle.

Electrophysiological and epicardial mapping studies are described in a patient without pre-excitation who had intractable recurrent paroxysmal supraventricular tachycardia. Electrophysiological studies revealed fixed VA conduction times during both rapid ventricular pacing and coupled ventricular stimulation. Catheter mapping of atrial activation during retrograde conduction and during induced ...
